- Person’s name + best way to reach them
- County/city they’re returning to (King/Pierce/Snohomish)
- Release date (or how long they’ve been home)
- Most urgent needs (housing, ID, job, treatment, transportation, mentoring)
- Any safety considerations we should be aware of
Step 3: We respond as soon as possible to schedule a brief phone intake and next steps.
